History Of Rhinoplasty
Rhinoplasty or nose job is not a new procedure . It is being done for 2500 years. It was started in India in 600 BC . It was mainly done for reconstruction following injury but for cosmetic reason or beautifying the nose was started in 1918. Initially was started by non surgical thing by giving appliances but real surgery was started in1936. Initially the results were not predictable but now with knowledge of anatomy and those who have experience like Dr S S Gambhir can give more predictable and satisfactory results. Rhinoplasty is an art and plastic surgeons are artist. They know how to handle the tissue to give good results. In delhi rhinoplasty is being done by both ENT and Plastic surgeon but i would suggest the doctor who is basically a Rhinoplasty surgeon will give a better result. Dr S S Gambhir has devoted his practice to rhinoplasty and the results speak of itself.
There has been always been debate regarding closed Rhinoplasty and open Rhinoplasty which one is best. As far as Dr Gambhir is concerned he prefer doing the procedure depending on the requirement . If there is minimal septal work and tip not to be addressed then close Rhinoplasty is preferred.
But when the tip plasty and septal deviation needs to be corrected then open Rhinoplasty is the method of choice.
The procedure involves carefully exposure of the tip which requires experience and lot of expertise. If the doctor is not experienced he can ruin the tip of it's support structures.
Then dissection of the dorsum up to the bone is required to visualize all the deformities and once assessed the sequence of correction need to be visualized and executed. Once the surgery is complete the surgeon check for any residual deformity if it is there it is corrected.
Dorsum is the key of Rhinoplasty we prefer doing DCF ( diced cartilage graft) which gives smooth contour to the dorsum.
The tip is sharpened if needed and domes defined. The angle can be corrected at this time to give a beautiful tip.
Once done we now no more pack the nose only splints are applied and patient can breath normally and he is comfortable post surgery . If bone work is done thermoplastic splints are applied.
The splints are removed after 10 days and then patient can see the result.
